Student Health and Development

Student well-being at Woodlands Academy is supported through a variety of programs aimed at fostering social-emotional development, healthy decision making skills, self-knowledge, and self-advocacy. Students are supported through their advisory, by opportunities for individual counseling, by college counseling, through participation in the Student Development Curriculum, and by the work of the Student Support Team.

Illinois School Health Record Requirements


Illinois state law requires all freshmen or transfer students not in compliance with the physical, eye, and immunization requirements be excluded from school beginning October 15th until the correct information is submitted to the School.

9th graders:

-Incoming freshman students must submit a physical examination (dated within 12 months of the first day of school) with all required immunizations prior to the start of school. This physical will be acceptable for participation in athletics for one year and 30 days from the date of the physical. Students may not participate in interscholastic sports or team practice until they have a physical on file.


-Sports physicals cannot be accepted as the required freshman physicals.

-Eye exams must be done for all students.

Transfer Students:

From an Illinois High School:

-Each student must present a copy of her 9th grade physical, or a more current, complete physical with updated immunizations prior to the start of school. A sports physical does not meet this requirement.

-Must complete the Illinois Eye Exam Form found on this web site

From another state:

- Each students must present a copy of her high school physical with updated immunizations prior to the start of school. If the previous high school did not require a physical, the student has 30 days to complete the Illinois State requirement and must be completed on an Illinois State form. A sports physical does not meet this requirement.


International Students:

Grade 9 students have 30 days from the start of the school year to complete the Illinois state requirements of a physical exam by a physician and proof of required immunizations. The physician must complete the Illinois State Form and a link to this form is listed below. A sports physical does not meet this requirement.

-All Grade 9 students must provide a completed Illinois Eye Exam Form found on this web site
Grade 10 or 11 students who are new to Woodlands Academy may submit a physical exam completed/dated within 12 months of the first day of school. The form submitted must contain all required parts of the Illinois State Form.

-All Grade 10 or 11 student who are new to Woodlands Academy must provide a completed Illinois Eye Exam Form found on this web site
